Health Services : Immunization Requirements
The following are the Texas immunization requirements for the 2007-2008 school year. (

3 year olds and 4 year olds (Pre-K and PPCD)
- DPT
- POLIO
- MMR
- 1 dose. First dose must be after 1st birthday
- HIB
- 3 doses with 3rd one on or after 12 months
- OR
- 1 dose after 15 months (not due when child turns age 5)
- HEP B
- VARICELLA
- 1 dose on or after 1st birthday
- Parent or physician validated history of varicella illness is
acceptable in lieu of vaccine.
- PNEUMOCOCCAL (PCV):
- At least 3 doses with one given on or after 1st birthday OR
- At least 1 dose after the 2nd birthday if child has never had immunizations (not due when child turns age 5)
- HEPATITIS A
- TB SKIN TEST
- Required for all students entering from a foreign country.
**There is a 4-day grace period for all vaccines.
Kindergarten – 12th Grade (2007-2008)
- DPT
- 5 doses of DPT OR
- 4 doses if 4th dose was on or after the 4th birthday
- POLIO
- 4 doses with 1 on or after the 4th birthday
OR
- 3 doses if the 3rd one was on or after the 4th birthday
OR
- Combination of 4 OPV and IPV received before 4th birthday
- MMR
- 2 doses. First dose must be after 1st birthday
- HEP B
- VARICELLA
- 1 dose on or after 1st birthday
- Parent or physician validated history of varicella illness is
acceptable in lieu of vaccine.
- TB SKIN TEST
