Emergency Care Cards
At the beginning of each school year, every student is asked to return a completed emergency care card. This is for your child's protection. Please inform the office of any changes in address or phone numbers. It is important that you are able to be contacted in the event of an emergency.
Medication
The Nurse's Office is open beginning at 7:30 each morning. All students are asked to bring any medication to the Nurse's office at that time. All medication must be in the original container and must be accompanied by a physician's release form. These forms are available from the nurse.
Immunizations
The new immunization law that became effective on August 1, 2000 is as follows:
Hepatitis B
All children turning 12 after September 2, 2000 must receive three doses of Hepatitis B vaccine by their 12th birthday. It takes six months to receive all three doses. Please plan ahead, and start early.
Varicella (Chicken Pox)
All children turning 12 after September 2, 2000 must receive one dose of varicella vaccine. If the child has already had the chicken pox, a form that includes the dates of illness from either the child's physician or the parents needs to be completed. These forms are in the nurse's office.
Measels, Mumps,and Rubella (MMR)
All children, prior to their 12th birthday, must have received at least two doses of the MMR vaccine. It can be administered anytime before the 12th birthday.
