State Required Immunizations for School Attendance

By law, the district must have an immunization record for each student on file. These records should be up-to-date at the time of registration. Records must show the month, day and year of each dose of vaccine and must be validated by a physician’s signature or a public health clinic card. Children entering Belton ISD for the first time must present written verification of the required immunizations at the time of enrollment.

Uninsured Students or Reduced Cost
Uninsured students can receive immunizations at a Bell County Public Health Immunization clinic. Click here for a list of locations that provide immunizations at a reduced cost.
I have my affidavit(s). Now what?

If a family chooses not to have immunizations, Texas public schools require one affidavit per child to be enrolled. The student can not begin school without a current, signed, notarized affidavit. Do not sign your form until you are in front of the notary.
Once notarized, the form must be turned in to the school within 90 days or it will become void.
Once notarized, the form is valid for 2 years.
Texas schools will accept only the official Texas Department of Health and Human Services (DSHS) affidavit forms. No other forms, photocopies or reproductions are valid.
Any child who utilizes the affidavit may be excluded from school in times of emergency or epidemic declared by the commissioner of public health.
